This website/service (http://www.mypaymentform.com/) is developed by CodeByRen and is provided primarily as a convenience for smaller and/or less technically-oriented merchants wanting to accept payments on their website (via the DPS payment gateway).

While CodeByRen will make the effort to keep the service running smoothly, there is absolutely no guarantee that MyPaymentForm is fit for the intended purpose or will meet any of your expected requirements.

MyPaymentForm reserves the right to suspend any new form creation in order to keep the service running smoothly. MyPaymentForm may also delete any forms that are deemed inactive (have not processed a successful transaction in 6-12 months). Of course, an attempt will be made to contact you via email first.

In the event that the person accepting these terms wishes to terminate the service(s) provided by MyPaymentForm, any active forms can be deleted by following instructions outlined here. Upon confirmation, the service will be terminated and any information collected will be deleted from the MyPaymentForm database.

MyPaymentForm does collect a minimal amount of information necessary to support the site's functionality. Accepting the terms and conditions outlined above indicates that you have also read, understood, and agree with the privacy policy.
